CVLI survey required to receive CVLI coverage

Churches who wish for CVLI coverage for the upcoming 2018-2019 year must complete the “2019 CVLI ministry with children” survey. It is included in the digital Charge Conference packet.
Submission is mandatory, and CVLI coverage cannot be included for the church without the completed survey. Attendance totals and the official name of the children’s ministry must be included for proper coverage.
Only one person from each church should submit a report. Questions about the form can be directed to the church’s district office, which oversees the Charge Conference forms.
